Job Overview
We are seeking a dedicated and detail-oriented Treatment Plan Coordinator to support our licensed mental health providers by coordinating, preparing, tracking, and maintaining individualized patient treatment plans in accordance with organizational standards, payer requirements, accreditation standards, and regulatory guidelines. This role serves as a key administrative and clinical support partner, ensuring treatment plans are completed accurately, on time, and reflect the clinical services being provided.

This position requires strong organizational skills and familiarity with medical terminology! The Treatment Plan Coordinator works closely with psychiatrists, nurse practitioners, and administrative staff to improve documentation quality, increase compliance, reduce provider administrative burden, and promote continuity of patient care.

Key Responsibilities:

  • Collaborate with clinicians to prepare individualized treatment plans based on documented clinical assessments and provider recommendations.
  • Draft treatment plans using clinician-provided goals, diagnoses, interventions, and measurable objectives.
  • Ensure treatment plans align with DSM diagnoses, medical necessity, and organizational documentation standards.
  • Maintain consistency in treatment plan language, formatting, and measurable outcomes.
  • Update treatment plans following clinician direction during required review periods.
  • Assist providers with revisions based on supervision, audits, or payer feedback.
  • Documentation & Compliance
  • Monitor due dates for initial treatment plans, updates, reviews, and annual renewals.
  • Track documentation completion and notify providers of approaching deadlines.
  • Review treatment plans for completeness before provider signature.
  • Verify required signatures are obtained from providers and patients when applicable.
  • Ensure documentation meets payer, state, accreditation, and organizational requirements.
  • Assist with internal chart audits and documentation quality improvement initiatives.
Qualifications:
  • Fluent in Spanish and English (preferred).
  • High school diploma or equivalent. 
  • Strong interpersonal and communication skills.
  • Ability to work in a fast-paced environment with empathy and professionalism.
  • Proficiency in basic computer and data entry skills.
  • Empathy and professionalism when interacting with patients.
  • Experience in mental health or healthcare settings is a plus, but not requiredwe provide comprehensive training.

Position Details:

Hours / Full-time
Compensation / $15 - $17 per hour, commensurate with experience/education
